01 / Challenge
Clinicians were finishing their notes after hours. Documentation ran long past the last appointment, and the parts that mattered most for continuity were the parts most likely to be written from memory.
02 / Approach
Consultation audio is transcribed as it happens and drafted into the clinic's own note structure. The draft stays a draft: nothing enters the record until a clinician has read and signed it, and every edit is kept.
The model never writes a diagnosis. It organises what was said, flags what it could not hear clearly, and leaves the clinical judgement where it belongs.
